Safe sex means taking precautions during sex that can keep you from getting a sexually transmitted infection (STI), or from giving an STI to your partner.

STIs are also referred to as sexually transmitted diseases, or STDs. These diseases include genital herpes, genital warts, HIV, chlamydia, gonorrhea, syphilis, hepatitis B and C, and others.

A sexually transmitted illness (STI) is a contagious disease that can be transferred to another person through sexual intercourse or other sexual contact. Many of the organisms that cause STIs live on the penis, vagina, anus, mouth, and the skin of surrounding areas.

Most of the diseases are transferred by direct contact with a sore on the genitals or mouth. However, some organisms can be transferred in body fluids without causing a visible sore. They can be transferred to another person during oral, vaginal, or anal intercourse.

Some STIs can also be transferred by nonsexual contact with infected tissues or fluids, such as infected blood. For example, sharing needles when using IV (in the vein) drugs is a major cause of HIV and hepatitis B transmission. An STI can also be transmitted through contaminated blood transfusions and blood products, through the placenta from the mother to the developing baby, and sometimes through breastfeeding.

The following factors increase your risk of getting an STI:

Not knowing whether a partner has an STI or not
Having a partner with a history of any STI
Having sex without a male or female condom
Using drugs or alcohol in a situation where sex might occur
If your partner is an IV drug user
Having anal intercourse

Drinking alcohol or using drugs increases the likelihood that you will participate in high-risk sex. In addition, some diseases can be transferred through the sharing of used needles or other drug paraphernalia.

Abstinence is an absolute answer to preventing STIs. However, abstinence is not always a practical or desirable option.

Next to abstinence, the least risky approach is to have a mutually monogamous sexual relationship with someone you know is free of any STI. Ideally, before having sex with a new partner, each of you should get screened for STIs, especially HIV and hepatitis B, and share the test results with each another.

Use condoms to avoid contact with semen, vaginal fluids, or blood. Both male and female condoms dramatically reduce the chance you will get or spread an STI. However, condoms must be used properly:

Keep in mind that STIs can still be spread, even if you use a condom, because a condom does not cover surrounding skin areas. But a condom definitely reduces your risk.
Lubricants may help reduce the chance a condom will break. Use only water-based lubricants, because oil-based or petroleum-type lubricants can cause latex to weaken and tear. Polyurethane condoms are less prone to breaking, but cost more than latex condoms. Using condoms with nonoxynol-9 (a spermicide) can help prevent pregnancy, but may increase the chance of HIV transmission because the spermicide can irritate the vaginal walls.
The condom should be in place from the beginning to end of sexual activity and should be used every time you have sex.
Use latex condoms for vaginal, anal, and oral intercourse.

Here are additional safe sex steps:

Be responsible. If you have an STI, like HIV or herpes, advise any prospective sexual partner. Allow him or her to decide what to do. If you mutually agree on engaging in sexual activity, use use latex or polyurethane condoms and other measures to protect the partner.
If pregnant, take precautions. If you have an STI, learn about the risk to the infant before becoming pregnant. Ask your provider how to prevent the developing baby from becoming infected. HIV positive women should not breastfeed their infant.
Know your partner. Before having sex, first establish a committed relationship that allows trust and open communication. You should be able to discuss sexual histories, any previous STIs or IV drug use. You should not feel coerced or forced into having sex.
Stay sober. Alcohol and drugs impair your judgment, communication abilities, and ability to properly use condoms or lubricants.

What is infertility?
Infertility is the term health care providers use for women who are unable to get pregnant, and for men who are unable to impregnate a woman, after at least one year of trying.

In women, the term is used to describe those who are of normal childbearing age, not those who can’t get pregnant because they are near or past menopause.

Women who are able to get pregnant but who cannot carry a pregnancy to term (birth) may also be considered infertile.
What causes infertility?
Infertility does not have a single cause because successful pregnancy is a multi-step chain of events.

Simply described, pregnancy includes the following steps: A woman’s ovaries must be able to release a viable egg, which then must be able to travel down the fallopian tube. The man must be able to ejaculate, and his sperm must be able to travel to the fallopian tube. The sperm and egg must unite to fertilize the egg. The fertilized egg must attach to the inside of a receptive uterus (or implant) and be nurtured by the body to allow the fetus to develop and grow until it is ready for birth.

Problems with any of these steps can lead to infertility.

The cause of infertility can rest in the woman or the man, or can be from unknown factors or a combination of factors. In some cases, environmental factors can contribute to infertility. In other cases, genetic conditions or other health problems are the main cause of infertility.

Male infertility

 

The male fertility process involves the production of mature sperm and getting the sperm to reach and fertilize the egg. Although it may seem to be a simpler process than female fertility, male fertility also requires many conditions to be met: the ability to have and sustain an erection, having enough sperm, having enough semen to carry the sperm to the egg, and having sperm of the right shape that move in the right way. A problem meeting any of these conditions contributes to infertility.

Like female infertility, male infertility can result from physical problems, such as testes that don’t make enough normal sperm, hormonal problems, and lifestyle or environmental factors, including (but not limited to):

Stress
Exposing the testes to high temperatures, which can affect the ability of the sperm to move and to fertilize an egg. For instance:

But, in some cases, health care providers cannot determine a cause for infertility in the man or woman. In addition, some known causes of infertility do not have any treatments.

Aging changes in the Female Sexuality

 

As a woman ages, a number of changes take place in the female reproductive system. For women, the cessation of menses (menopause) is an obvious sign of aging. But, it is by no means the only change. A transition period, called the climacteric, lasts for many years before and after a woman's last menstrual period.

For a woman, aging changes involve hormone levels, physical changes in the woman's entire reproductive tract, and psychological changes. Changes occur in the intricate relationship between the ovarian hormones and those produced by the pituitary gland (in the brain).

AGING CHANGES

Menopause is a normal part of a woman's aging process. The ovaries stop releasing eggs (ova), and menstrual periods stop. Most women experience menopause around age 50, although it occurs before age 40 in about 8% of women. Prior to menopause, menstrual cycles often become irregular.

The ovaries become less responsive to stimulation by follicle-stimulating hormone (FSH) and luteinizing hormone (LH). To try to compensate for the decreased response, the body produces more of these ovary-stimulating hormones for a time. The level of these hormones will eventually decrease.

The hormones produced by the ovaries include the different forms of estrogen (including estradiol), progesterone, and androgens (including testosterone). These hormones also decrease around menopause. The ovaries continue to produce small amounts of testosterone and some estrogen. The hormones produced by the pituitary gland are also decreased.

Because hormone levels fall, changes occur in the entire reproductive system. The vaginal walls become less elastic, thinner, and less rigid. The vagina becomes shorter. Secretions become scant and watery. The external genital tissue decreases and thins (atrophy of the labia).

In both men and women, reproductive system changes are closely related to changes in the urinary system.

EFFECT OF CHANGES

Prior to menopause, fertility varies depending on hormone levels. Menopause is said to have occurred when there has been one year without a menstrual period. With menopause, reproductive capacity is lost (the woman can no longer become pregnant).

Symptoms that accompany menopause vary in severity. Many of the symptoms are triggered by the hormone changes. Only about 20% to 30% of all women have symptoms that are severe enough that they seek medical attention.

Changes occur in the woman's breast tissue at the same time as other reproductive changes. A woman may experience changes in her sex drive (libido) and her sexual response may change, but aging does not prevent a woman from being able to have or enjoy sexual relationships. Most often, things such as availability of a partner, symptoms from vaginal dryness and thinning vaginal walls, and psychological and social factors affect a woman's sexual response more than direct aging changes.

COMMON PROBLEMS

The pubic muscles lose tone, and the vagina, uterus, or urinary bladder can fall out of position. This is called vaginal prolapse, bladder prolapse, or uterine prolapse, depending on which structure drops. A prolapse of any of these structures increases the risk of problems such as stress incontinence (urine leakage). Most prolapses can be treated.

Irritation of the external genitals can occur (pruritus vulvae). The vaginal walls become thinner and dryer and may become irritated (atrophic vaginitis). Sexual intercourse may become uncomfortable for some women (dyspareunia). There are changes in the levels of normal microorganisms in the vagina, and there is an increased risk of vaginal yeast infections.

Similar changes to the bladder and urethra may increase symptoms such as frequency and urgency of urination, and there is an increased risk of urinary tract infection after menopause.

Hot flashes, mood disturbances, headaches, and sleep disturbances are also common symptoms that occur during menopause. The causes of these changes are not well understood, but they are also related to the decreasing amount of estrogen produced by the ovaries.

Osteoporosis risk is greater in older women. This is caused, in part, by decreased estrogen levels.

PREVENTION and TREATMENT

Adequate lubrication (with a water-soluble lubricant) can help prevent painful sexual intercourse. A topical estrogen (applied inside the vagina) is sometimes recommended to help maintain the structure of the vaginal tissues. Sexual changes can also be related to factors other than simple aging.

Estrogen replacement (such as pills or patches) can prevent or reduce many of the symptoms associated with menopause, including vaginal atrophy and hot flashes. It also decreases the risk of complications such as osteoporosis.

However, hormone therapy (HT) may have side effects such as vaginal bleeding, and it has been associated with an increased risk of breast cancer, stroke, and heart disease, depending on when it is started and how long it is used. The risk and benefit of treatment is different for each person so women should discuss the pros and cons of estrogen treatment with her own health care provider.
